COVID-19 Vaccination Reporting for Students

Over the summer months, Lindbergh Schools is requesting that families please continue to notify us when your student is fully vaccinated for COVID-19. At this time, the Pfizer vaccine has been approved by the FDA for emergency use for ages 12-18.
Please complete the Lindbergh Student COVID-19 Vaccination Reporting Form when your student is fully vaccinated for COVID-19, and email a picture of their vaccine card to your school nurse. Your student's health information will be kept confidential.
General, unidentifiable, data gathered on this form may be used to help guide district decisions for the safety and wellbeing of Lindbergh students and staff, in cooperation with the St. Louis County Department of Public Health.
COVID-19 Summer Protocols
Lindbergh Schools will continue to follow CDC and St. Louis County COVID-19 guidance. Beginning on June 1, 2021, the following procedures will be observed for all summer student programming. These procedures are in place to protect the large number of students who will not be fully vaccinated during these summer months.
- All students and adults must continue to wear face coverings indoors, anytime student programming is taking place.
- Students and staff may remove face coverings while outside, as long as they practice social distancing.
- Face coverings must be worn on the bus.
- Students and staff should continue to self-check for symptoms at home using the Daily Health Screening Form.
- Lindbergh’s COVID-19 dashboard will resume on June 14 and continue to be updated weekly during summer school.
