COVID-19 Vaccine Community Clinic Set for April 15

We are pleased to announce that Lindbergh Schools has partnered with Pharmax Pharmacy to host a COVID-19 vaccine community clinic Thursday, April 15 from 1-6:30 p.m. at Lindbergh Schools Central Office.
This clinic is open to the public, but pre-registration is required. Registration will close at the end of day Wednesday, April 14.

Eligibility Requirements
This clinic is open to the public, but pre-registration is required. Individuals must:
- Be 18 years of age or older
- Have not tested positive for COVID-19 in the past 14 days
- Have not received antibody therapy in past 90 days
- Have not had an allergic reaction to any component in the vaccine
- Have not had any type of vaccination in past 14 days (ex: tetanus shot)
How to Sign Up
Please complete the COVID-19 Vaccine Community Clinic Signup Form to reserve your appointment. Pre-registration is required.
What to Bring
Please bring the following items to your appointment. Vaccines will not be administered unless you provide all of the following information:
- A COVID-19 vaccine consent form. This must be signed and completed prior to your appointment.
- A photocopy of your driver’s license or photo ID (front and back)
- A photocopy of your insurance card (front and back). If you do not have insurance, you can still receive the vaccine at no cost.
- Note: Access to copiers will not be available on site. Please make a paper copy of your identification cards prior to your appointment date.
In order to ensure adequate parking, please do not arrive more than five minutes early for your appointment. Please plan on staying 15 minutes after your appointment to be observed.
In addition, please read the Moderna COVID-19 Vaccine Fact Sheet prior to your appointment. If you have questions about COVID-19 vaccinations, please contact your healthcare provider and visit the Missouri Department of Health COVID-19 vaccine facts website.
Important Notes
To ensure that all doses are administered during the clinics, it is very important that you do not sign up for a COVID-19 vaccine anywhere else once you have made an appointment with Lindbergh Schools. If you are unable to make your appointment, please cancel it before April 15.
Everyone will receive the Moderna vaccine, which requires two doses. Pharmax pharmacists will return to Central Office to administer second doses of the vaccine approximately four weeks after the initial clinic date. Lindbergh Schools will send out additional information about the second vaccination after the first vaccine is completed, but this date is tentatively scheduled for May 13.

FEMA Community Vaccination Event
If you are unable to attend Lindbergh’s community clinic, Lindbergh Schools has been asked to share information with the community about the following mass vaccination event. In partnership with MO DHSS, FEMA is holding a Community Vaccination Event at the dome in downtown St. Louis City. The clinic event is daily from 8 a.m.-6 p.m. Additional days and times will become available for scheduling as supplies are confirmed.

Registration is required. Please visit the Missouri COVID-19 Vaccine Navigator website to sign up. After registering, you will be emailed scheduling links as the events are published in your area.
Lindbergh Schools is not responsible or in charge of this additional vaccination event. We have no further information on what type of vaccine they will be giving or how to schedule an appointment. Please do not sign up for both events. We are sharing this information with you in case you are unable to attend Lindbergh’s community clinic.
